Consumer Directed Services (CDS) Quarterly Financial and Service Reports and Annual Service Reports are now contracted with DocuSign

Missouri Medicaid Audit & Compliance (MMAC) Provider Review recently contracted with DocuSign to make it more efficient for you to complete and submit the required CDS Quarterly Financial and Service Reports and the Annual Service Reports.

The reports are still located at the same link as before (see link below), but now when you click on the link for the report you are wanting to fill out, it will take you to DocuSign to fill out the report.

Please note that when you utilize DocuSign for filling out the reports that the email address you submit before entering into the report will input into the report automatically and should be the provider agency email and not the individual who is filling out the reports business email.

Beginning with all 1^st Quarter 2024 Reports that are due by April 30, 2024, MMAC will require CDS providers to utilize DocuSign for these reports and all future reports submitted.
